New Patients
Begin your care with us.
A few thoughtful steps to complete before your first appointment.
Before your first appointment
Complete your new patient packet in advance.
Your completed new patient packet must be submitted at least 2 business days before your appointment so your provider has adequate time to carefully review your history, medications, symptoms, and treatment needs.
Please do not wait until the day of your appointment to complete the packet. Packets submitted late may not provide enough time for clinical review and may require the appointment to be rescheduled.
Please arrive 20 minutes early for your first appointment so we can complete your vitals, including height, weight, and blood pressure, before your provider joins you.
Packet completion will open here once your first appointment is scheduled.
Business days do not include weekends or holidays.
Thoughtful preparation
Your visit begins before you arrive.
Reviewing your information beforehand allows your provider to prepare thoughtfully and reserve your scheduled appointment time for a focused clinical conversation.
Your packet also helps confirm important medical history, current medications, previous treatment, and the concerns you would like to address.
Arriving for your visit
A few things to bring, so your first visit can begin with ease.
Thoughtful preparation lets us spend your appointment listening and caring, rather than gathering details. Please plan to arrive a little early and bring the following.
Arrive 20 minutes early
This allows time for vitals, including height, weight, blood pressure, and pulse, and a calm check-in before your provider joins you.
Photo ID & insurance card
Bring a current photo ID and, if applicable, your insurance card so we can confirm your details at the front desk.
Current medications & supplements
A list of every medication and supplement you take, including dosages, helps your provider review your care safely.
Your pharmacy
Bring your pharmacy name and address so any prescriptions can be sent promptly after your visit.
Prior records & labs
If you have previous treatment records or recent lab results, bring them along. They help complete the picture.
Plan for the full appointment
First evaluations often run longer than follow-ups, so avoid scheduling anything tight immediately afterward.
